Transition Planning

Watching our students grow up is a time of celebration for us, but can be a challenging time for family and care givers. The staff of Benedictine is devoted to ensuring every transitioning youth is as independent as able at the time of graduation, and every student graduates to adult services that best matches his or her achievements, interests, and needs. 

Preparation for this important time begins many years before graduation day. Transition planning for the student begins at age 14 and involves:

  • Increased application of skills in a variety of community settings
  • Provision of OT/PT and speech in natural settings per the IEP
  • Vocational training on campus and in the community
  • Facilitation of a number of assessments and evaluations that help prepare for and guide recommendations for services as an adult
  • 24/7 (if residential) team support & reinforcement of daily living, social, and communication skills for optimal independence and self-determination—for day students, collaboration with care providers to promote reinforcement

To help parents navigate the transition from school to adult services, Benedictine works collaboratively with students, family, IEP team and key professionals to address concerns and answer the many questions that arise while preparing each transitioning student for success.  Some questions are:

  • When and how should I start to look for adult services?
  • How will my child be prepared for adult living?
  • What services are available?
  • Is funding available?
  • What are the deadlines?
  • How do I determine the best services?
  • Who will help with the application process?

 Our transition team supports each family and student every step of the way.
